Submission of Application Requirements

To all applicants who have successfully completed the Google Form and created an account on the Enrollment Registration System (ERS):

You are required to submit the following documents in person:

  1. Grade 11 Report Card (Photocopy)
  2. Printed ERS Application Form (downloaded from your ERS account)

Submission Period:
Submission of documents will be from September 29, 2025 to October 10, 2025, at the Office of Admissions, from 8:00 AM to 4:00 PM, Mondays to Fridays only.
