SCHOLARSHIP PRIVILEGES UPON ENROLLMENT

For students availing the following scholarships, the following documents must first be submitted to the Office of Student Affairs (OSA) for scholarship notation before assessment of fees.

1. Entering Honor Student (Valedictorian/Salutatorian)
   • Certification
   • Graduation Program
2. Barangay Grant or dependent of Barangay Officials
   • Certification of Oath of Office (DILG & Mayor)
   • Affidavit of Support
   • Income Tax Return of Barangay Official
   • Birth Certificate
3. Sangguniang Kabataan (within the NCR only)
   • Certification of Oath of Office (DILG & Mayor)

SCHOLARSHIP REQUIREMENTS

1. For new students
   • Photocopy of High School Card
   • Certification of Good Moral Character
   • Photocopy of Income Tax Return of Parents/Guardian/Affiadavit/Certificate of Non-filing
   • Birth Certificate
   • 1 1/2" x 1 1/2" Picture
2. For higher year
   • Original/photocopy of checklist/class cards
   • Photocopy of Income Tax Return of Parents/Guardian/Affidavit/Certicate of Non-filing
   • 1 1/2" x 1 1/2" Picture
